Gweithiwr Cymdeithasol - Tîm Plant Sy'n Derbyn Gofal X 2 /social Worker - Children Looked After Team

The social worker will co-produce proportionate assessments and Care and Support Plans with families and partner agencies, produce court reports and ensure that children looked after experience permanence which supports them to achieve personal outcomes and make a successfully transition to adulthood. The social worker will hold a case load supporting Children Looked after, children whose names are on the Child Protection. The post requires some court work to respond to applications to court for issues such as contact and for reasons of revocation

The post holder will pursue enquiries with regard to allegations of child abuse (Children Act 1989 Section 47) and follow Cwm Taf Safeguarding procedures and the All Wales Child Protection procedures.

Safeguarding children within our community, working with children/young people and their families / carers to achieve personal Well-being outcomes. Supporting families, in collaboration with other key professionals, through change and signposting to services within the community.
