Job Description
Purpose of the post • Ensure that the people of Gwynedd are at the heart of everything we do.
• Contribute towards providing Admissions Team service to early intervention referrals within the department.
• Helping to receive inquiries and referrals from Families First / Early Intervention on behalf of the department from service users, their carers and families or other professional officers.
• Hold "What's Important" discussions with children and families, assess their needs and record their wishes.
• Ensure that children and families are directed to advice, information and the most suitable services for them.
Responsibility for functions • Computer equipment i.e. lap top, mobile phone, projector
Main duties • Support the Families First Referral Reception Officer to respond to inquiries for information and receive Early Intervention referrals from families and a variety of sources. Record them in accordance with guidelines and respond to the references in accordance with the procedure and enter the information on the Department's WCCIS system.
• Hold discussions with children and their families to complete an assessment of their needs and a "What's important" conversation. This can happen over the phone, virtually or in person.
• Listening to, and collecting, all suitable and relevant contact inquiry information from the customer in order to facilitate and advise further services.
• Refer children and families who need early support to the most appropriate service for them at the time.
• Collaborate with key partners such as education, health and the 3rd sector to discuss referrals and collect relevant information.
• Dealing with phone, Email and paper inquiries and providing general advice and information.
• Dealing with and resolving inquiries or addresses from start to finish, and being responsible for all the activity involved in resolving an inquiry or referral.
• Liaise with statutory, non-statutory and 3rd sector services across the County in order to gain an understanding of the support available to families in Gwynedd and to promote early intervention work.
• Working in partnership with services to ensure the best support for families and identify any gaps.
• Collaborate and network with other officers and partners relevant to inquiries and inquiries from service users.
• Support information sharing sessions to raise the profile of Gwynedd Families First and how to refer cases.
• Record and update information onto WCCIS. Maintain appropriate computer systems.
• Work in a flexible manner.
• Work in a caring and sensitive manner.
• Assist with monitoring and monitoring the quality of the service.
• Improve customer satisfaction and the provision of service; contributing ideas for developing and promoting the service, identifying improvements to the processes, collaboration and the Services.
