You will assess and support children, young people (0-25) and families in variety of settings including their home, schools, and communities. Your assessment will be outcome focused, recommending and implementing intervention that are least restrictive to reduce the impact of disability and promote independence.
In order to achieve positive outcomes for children, young people and families, you will be engaging with housing grant departments, health, education and other disability and equipment services.
