Home Care and Reablement Campaign

Between 18 November - 8 December, WeCare Wales are shining a light on home care and reablement. We will be sharing stories on our social media channels, learning about the variety of roles available and looking at the importance of these services.

Home care services help individuals stay connected to their communities, family, and friends. It's personalised care for those recovering from surgery, or with disabilities. Working in this field allows you to make a difference in people's lives by helping them remain safe and independent at home.

To support our campaign we will be running our Get into sessions.

The free hour long webinars are suitable for anyone living in Wales wanting to learn more about home care or reablement. See below for more information and how to book your place.

Get into: Home care

A free webinar on 28 November between 1:30pm and 2:30pm for people living in Wales who are interested in learning more about how to get into home care.

Get into: Reablement

A free webinar on 5 December between 10:00am and 11:00am for people living in Wales who are interested in learning more about how to get into reablement.

Karima's story

Karima is a dedicated home care worker for over 15 years. Originally from Libya with a degree in Geology, she found the perfect fit in care work as a single mother.

Bill's reablement story

We co worked with RCT’s reablement team to get a taste of the reablement service in the area. Take a look at our new video featuring Bill, who shares his inspiring story of how the reablement service supported him to regain his independence following a fractured femur.

50 day challenge

The Welsh Government has launched a 50-day challenge aimed at improving hospital discharge and community care to ease winter pressures on the health and care system

This initiative, starting on November 11, 2024, involves health boards and local authorities working together to help patients return home safely and promptly.

The plan aims to:

  • Improving hospital discharge procedures and planning for discharge from the point of admission.
  • Implementing seven-day working to enable weekend discharges.
  • Providing community rehabilitation and reablement to support recovery at home.
  • Establishing integrated navigation hubs to support hospital discharge and avoid community admissions

The challenge targets those who have experienced long hospital stays, aiming to reduce delays and improve patient outcomes by promoting best practices across Wales

This effort is part of the broader winter resilience plans, supported by significant funding to build community capacity and ensure effective care transitions.
