Job Description
Purpose of the post • Ensure that the people of Gwynedd are at the heart of everything we do.
• Support and encourage service users to be as independent as possible by assisting the individual their families and carers with tasks they cannot undertake or are finding it difficult to do on their own.To ensure that the physical and emotional wellbeing of service users are met by implementing the specific Service User Plan.This is done by following all guidelines and policies as well as compliance with Gwynedd Council and the relevant National Standards and Regulations of the Care Act 2000.
Responsibility for functions • Respect and use appropriately any tools and equipment required to fulfill your duties
• Notify your Line Manager and / or the person responsible for the equipment as soon as possible of any defect therein, or the lack of services (water, electricity, gas etc.
• Ensure appropriate management of service users property e.g. medication and cash.
Main duties • To ensure and respect the dignity and independence of service users through good care practices and in accordance with Gwynedd Council Policy National Standards and Regulations of the Care Act 2000
• To provide support and carry out specific tasks in accordance with the Individual Care Plan e.g. personal care
• To be sensitive to the need to respect the rights, beliefs and preferences of the individual at all times.
• To behave professionally at all times and comply with the appropriate dress code
• To communicate effectively with service users, families, staff and other agencies and respect confidentiality at all times.
• To be part of the planning, preparation, implementation and review of the Service User Plan.
• To act as a key worker if applicable for a set number of service users and attend review meetings and case conferences of those service users and contribute written and verbal report.
• To record information in documents and service user's files on a daily basis.
• To undertake cleaning and laundry duties
• To prepare meals and drinks as required and serve meals.
