Support Worker - Supported Tenancies Job description About the role: 1.Post is a 21 hour per week Support Worker
Working to a six-week rolling rota subject to change to meet needs of the service. • There is a requirement to work shifts day and evenings also weekends and Bank Holidays. You will be supporting Adults who have a Learning Disability to manage their own home as to the principal responsibilities detailed in the Job description. About you: • We are looking for someone who has a positive attitude and a passion for working with people who have a Learning disability. • Be able to deliver a service that is person centred has a strong focus on positive outcomes and an increase in people's wellbeing. • To have an outgoing and friendly attitude with a positive outlook and a desire to meet all required challenges and aspects of the role. • To be able to think outside of the box and find creative solutions. • To always embrace and deliver the corporate values in everything we do these being. Professional, Positive Progressive, Open, Collaborative, • To have the skills and knowledge on how to treat people with learning disabilities as valued individuals who have worth and dignity, with equal rights to a full place in society and encourage others to do so. • To be the holder of a full clean driving licence What you will do: • Supporting people with Learning disabilities to develop their skills and independence by demonstrating and modelling activities. • Working alongside a dedicated staff team and other professionals in the service users' own home and other required venues. • Undertaking practical tasks and supporting people with their personal care, domestic activities and socially in the community including driving service users to activities and appointments. • Supporting service users to build and maintain family and friendship links and to reach goals and achieve personal outcomes. • Participating constructively in regular staff supervision and appraisal sessions. • Contribute to written assessments and Care Plans and attend Care Planning meetings compiling and keeping detailed records and reports. • Participating/attending required training to develop, grow and gain skills to enhance the role.
