Job Description
Purpose of the post • Ensure that the people of Gwynedd are at the heart of everything we do.
• To ensure that families which need additional support, within the Arfon Area, receive an integrated and co-ordinated preventative service, which is effectively planned on the basis of a partnership between the family and the agencies which work with them.
Responsibility for functions • Management of resources which are allocated to support the work i.e. lap top, projector, mobile phone, training packs
Main duties Area Co-ordinator Role
• Engage and build an effective working relationship with families who are referred to the Gyda'n Gilydd Team in order to plan interventions which are tailored around the needs of the family.
• Establish, maintain and support an effective working relationship with a wide range of services (statutory, non-statutory and 3 rd sector) which work with children, young people and families within Arfon, the Communities First Areas and beyond, to ensure a good working relationship with the Gyda'n Gilydd Team.
• Provide guidance, advice and information on the operational policies of the Gyda'n Gilydd Team to ensure a widespread understanding of the guidelines on referral criteria, undertaking Family Assessments, and the role of the Family First Allocation Meeting.
• Undertake Family Assessments and adhere to the "distance Travelled" measuring tool that has been devised for the Team: it measures the effectiveness of the team Around the Family service on different aspects of the family members' wellbeing e,g. income, housing, emotional health, the children's education, employment etc.
• Chair meetings between families and professional workers to create action plans. Arrange and chair meetings to monitor the action plans. Plan for bringing the intervention of the Gyda'n Gilydd Team to an end.
• Respond sensitively to the language needs of families and strongly adhere to anti discriminatory practice.
• Participate in training workshops for staff, including Family First service providers, to include the important operational elements of the Gyda'n Gilydd Team (i.e. Information sharing protocols, Family Assessment, multi-agency and integrated working and Gyda'n Gilydd Meetings).
• Share information with the Gyda'n Gilydd Manager regarding any developmental elements that arise. Be happy to discuss and resolve any barriers to co-working.
Strategic Role
• Take responsibility as a lead in a specialist field or fields within the team e.g. age group (early years/ 4-11/ 11-16/ 16-25) or in a specialist subject e.g. substance misuse, mental health, homelessness.
• Take part in the process of developing and reviewing Gyda'n Gilydd policies.
• Attend Staff Meetings and other meetings at the request of the manager
• The postholder is expected to be aware of the requirements of the Health and Safety legislation and to cooperate with the Directorate's Health and Safety Manager to ensure compliance with the relevant requirements.
