Reablement Practitioner - Integrated Reablement Service

The service provides reablement to people living in Torfaen to support them to live independently, safely and well in their own homes or within short stay accommodation. The service is flexible and responsive.

Your role as a Reablement Practitioner offers a unique opportunity to work as part of a multi-professional team. To use your knowledge, skills and attributes to assess, enable, support, supervise and assist individuals following illness and\/or hospital stay to maximise and maintain their independence in a way that is focused on what matters to them and their personal outcomes.

This service operates between 07:00-23:00 7 days a week including bank holidays and will involve weekend working on a rota basis in line with Torfaen Councils Domiciliary Care Service.
