About the job The postholder will be responsible for identifying and assessing the well-being needs of the whole family, but specifically with a focus on the child(ren) or young person with an identified disability or additional need. They will directly deliver short-term interventions, provide disability related advice and ensure that the family are fully aware of the support services available to them. The aim is to empower families to identify and use their strengths, to ensure that all necessary support is in place to prevent children and young people from requiring intensive assistance or escalation to statutory services.

- Proven experience of building effective working relationships with a diversity of individuals and groups
- A comprehensive understanding of the complex needs of children, young people and their families, particularly how additional needs and disabilities will impact on a child or young persons ability to meet key milestones and manage behaviours and emotions.
- A comprehensive working knowledge of the Microsoft Office package and of case management systems.
The right individual will also be able to recognise whether an individual is at risk and be able to consult and liaise with Social Workers and with other professionals or partner agencies as required.
