Come and join our team The carers assessment coordinator post will undertake statutory carers assessments, provide information, advice, and assistance to unpaid carers and will link in with 3rd sector organisations to look at ways to support unpaid carers. The carers assessment coordinator will attend GP cluster meetings and will accept referrals through that meeting. Applicants should be motivated and experienced individuals who have the ability to work in partnership with a range of organisations, services and health professionals to produce assessments, develop care and support plans, reviews and direct work that are person centred and outcome focused.
Applicants should be conversant with relevant legislation and statutory obligations and will need to be able to demonstrate an active commitment to the principles of promoting independence for people with disabilities.
The position is linked to Carmarthenshire's Long Term Social Work Team covering adults with physical disabilities and older adults with cognitive difficulties. This post is based in Carmarthen and you will be supporting residents who reside in the Carmarthen Town, Whitland, St Clears, St Peter's, Carmarthen; Furnace house, Morfa Lane, Meddygfa Tywi, Nantgaredig Meddygfa Teilo, Llandeilo Llanfair surgery and Llandovery.
