Come and join our team Main Purpose of role
To promote independence and safeguard older and disabled adults, who appear to have care and support needs, in the County.
To contribute to the work of the Community Resource Teams, and, when appropriate, in the acute and community hospitals. Undertaking assessments, providing information and advice, care and support planning and review under the supervision of the Assistant Team Manager, in line with the principles of the Social Services and Wellbeing Act Wales and all other relevant legislation

DBS level: An Enhanced with Barred List Disclosure and Barring Service (DBS) check will be requested for this job. It is an offence to apply for the role if the applicant is barred from engaging in regulated activity relevant to children or adults at risk
Welsh language level - Verbal: Level 2 - You will need a basic level of conversational Welsh. Reasonable support can be provided on appointment to reach this level.
