Job Description
Purpose of the post
• Ensure that the people of Gwynedd are at the heart of everything we do.
• To ensure that Gwynedd families have a voice, choice and control in the way in which they're supported to overcome barriers to reach their potential in relation to the aims of the Social Services and Wellbeing Act (Wales) 2014.
• Ensure that families in the Arfon area, who need extra support, receive a preventative service that's integrated, co -ordinated and that has been effectively planned on the basis of partnership between the families and the agencies working with them.
• Work with children and families that are have been affected by, or an in danger of being affected by Adverse childhood Experiences (ACEs), to help stop this or mitigate the effect of exposure to them to help break the intergenerational cycle of adversity.
Responsibility for functions
• Management of resources which are allocated to support the work i.e. lap top, projector, mobile phone, training packs
Main duties
Area Co-ordinator Role
• Engage and build an effective working relationship with families who are referred to the Team Around the Family. Work with them and other agencies supporting them to plan interventions which are tailored and targeted around the needs of the family.
• Establish, maintain and support an effective working relationship with a wide range of services (statutory, non-statutory, 3rd sector and services commissioned by Families First), to ensure a good working relationship with them and offer families a seamless service to help them reach their wellbeing outcomes and needs.
• Provide guidance, advice and information on the operational policies of the Team Around the Family to ensure a widespread understanding of the guidelines on referral criteria to The Intake and Referral Team (IAA)and form family assessments.
• Undertake Care and Support Assessments and adhere to the "distance Travelled" measuring tool that has been devised for the Team Around the Family.
• Keep up to date and correct information about families on the WCCIS data base.
• Chair meetings between families and professional workers to create action plans and meetings to monitor the action plans. Plan for bringing the intervention of the Team Around the Family to an end when a family case closes.
• Respond to enquiries from professionals when there's a safety concern about a child or family that are open to the Team Around the Family. Discuss concerns further with the Team Around the Family Manager and the manager within the Children's Intake and Assessment Team when there are concerns about whether the case needs to be "stepped up" to The Duty and Assessment Team or not.
• Work alongside statutory children's services for periods of time when cases are in the process of being "stepped up" or "stepped down" to preventative Team Around the Family service.
• Refer families on to services that are funded by Team Around the Family and other community services available with the family's consent.
• Respond sensitively to the language needs of families and strongly adhere to anti discriminatory practice.
• Participate in training workshops for staff, including Family First service providers, to include the important operational elements of the Team Around the Family.
• Share information with the Team Around the Family Manager regarding any developmental elements that arise. Be happy to discuss and resolve any barriers to co-working.
Strategic Role
• Ensure that the Gwynedd Families First Programme contributes towards the strategic aim of the Children and Families Team to improve the alignment of the Families First support to that of statutory provision in Gwynedd.
• Take part in the process of developing and reviewing TAF policies in times of change.
• Attend Staff Meetings and other meetings at the request of the manager.
• The post holder is expected to be aware of the requirements of the Health and Safety legislation and to cooperate with the Directorate's Health and Safety Manager to ensure compliance with the relevant requirements.
• Responsibility for self - development.
• Ensure compliance with Health and Safety rules in the workplace in accordance with the responsibilities noted in the Health and Safety at Work Act 1974 and the Council's Health and Safety Policy.
