An exciting opportunity has arisen in the Adult Disability Team for a Social Worker – Transitions – based in North Powys. We would like to hear from you if you are interested in working in partnership with young people, their families, carers and supporters, to enable a smooth and consistent transition into adulthood, and to also be part of team that is forward-thinking, supportive and uses a strengths-based approach to support people to achieve positive outcomes.

We are looking for a social worker who will be enthusiastic and creative, committed to collaborative working across agencies to ensure seamless care pathways which safeguard and promote independence, and who will focus on what matters to the young person. We believe that by exploring people’s goals and aspirations, their strengths and networks of support, we can help people to stay independent for longer and connected to their communities.
