We are looking for Community based Support Workers to join our M&D Care Community Outreach team, who will assist us in opening doors to a more positive future for the people we support. You do not need experience, just enthusiasm and compassion to support vulnerable adults in the community. *Paid for full shift worked, including down time between calls and travel time between homes.
*Part-time and Full-time contracts available from 22.5 to 45 hours per week.
As a community-based support worker you will provide essential care and support for people who live in their own homes. This support is built around people’s needs and routines to help them remain in their own home in a safe and supported way. Helping people stay connected with their communities is one of the most important aspects of this service. Domiciliary care enables people to keep doing the things that matter most to them while staying connected to their friends and families.
You will become part of our incredible team, helping to deliver emotional and practical care and support for vulnerable adults and transforming their lives on a daily basis.
You will be involved in providing quality care to vulnerable adults in the community, you will ensure the highest quality of care is provided to the people supported through their person-centred care plans.
You will need to be able drive (or – depending on where you’re based – willing to use public transport to travel between homes).
You will be working on a rota basis, which may include early starts, late finishes and day and night shifts and weekend working.
You will offer focused support with a range of tasks, including personal care, preparing meals, administering medicine, shopping, and light household duties.
You will be involved in supporting the daily activities of the people we support both in their own home and within the community.
You will be involved in facilitating activities which are person-centred, which promote their dignity, independence and well-being.
You will need to be a positive and caring person, who is confident and able to deliver individualised care and support.
You will need to be reliable, dependable and committed to making a difference.
You will need to have the desire and passion to support individuals to achieve their aspirations and personal goals.
You will need to demonstrate a commitment to enabling people to live fulfilled and independent lives both within their homes and the wider community.
You will need to be committed to providing an excellent level of care for vulnerable people with mental health and learning disabilities.
You will need to be an energetic, happy person who enjoys forming relationships with all sorts of people.
You will need to be over 18 to apply for this role.
If not already qualified, you should be working towards your QCF level 2 in Health and Social Care qualification. Proven experience in care advantageous but not essential as full induction and training will be provided.

Ability to be paid the National Living Wage (£9.30ph) after 12 months, if set criteria met
