Hours: 12-hour shifts | Days, Nights & Weekends (Rota Basis)
Grade 4 - £25,185 for 30.5 hours per week for 52.14 weeks per annum equating to £20,760.61 pro rata.
We are looking for a motivated and compassionate Reablement Support Worker to join our friendly, supportive team at our 8-bed Reablement Unit in Pembroke Dock, covering 12 months' maternity leave.
This is a rewarding opportunity to work within a welcoming Multi-Disciplinary Team (MDT), supporting individuals to regain independence, rebuild confidence and return home safely following illness, injury or hospital admission.
About the Role As a Reablement Support Worker, you will:
- Provide person-centred support to individuals during their reablement journey
- Encourage and enable independence with daily living skills
- Work collaboratively with colleagues across the MDT, including therapists and healthcare professionals
- Support individuals with personal care, mobility, nutrition and emotional wellbeing
- Maintain accurate records and contribute to care planning and reviews
About You - We are looking for someone who is:
- Caring, patient and respectful
- Passionate about promoting independence rather than doing tasks for people
- A strong team player with good communication skills
Flexible and reliable, able to work 12-hour shifts including days, nights and weekends Experience in health or social care is desirable, but not essential - full training and support will be provided.
