Job Description Purpose of the Post. • Ensure that the people of Gwynedd are at the heart of everything we do.
• Act on the realisation of personal outcomes and the promote the well-being of children and young people in accordance with the principles of the Social Services and Well-being Act 2014.
• Lead, co-ordinate and provide support services for children and young people who are open to the Derwen Integrated Team.
• Work with children and young people, family/guardians and other key workers to identify 'what matters to them' and find creative ways to meet their personal outcomes.
• Ensure that the service is inclusive and able to respond to a range of different needs.
• Provide support within a person-centred ethos, with a focus on promoting well-being and opportunities to use facilities within the local community.
• Lead on a service that promotes skills development and independence within what is known as the child/young person's attainment.
Responsibility for functions . e.g. staff, budgets, equipment • Responsibility for managing staff.
• Responsibility for monitoring and managing the Resources budget and staffing structures of the Service.
• Responsibility for equipment and vehicles used by the Service and ensure that these are in good condition and fit for purpose.

Main Duties • Lead and implement the provision of person-centred services.
• Assess and plan individual packages following an ethos of active support.
• Draw up and review general, specific and specialist risk assessments in collaboration with members of the multi-disciplinary team.
• Produce bespoke and specific programmes that meet the needs of the child/young person and review them regularly.
• Lead on Domiciliary Care packages which is a registered service. Ensure that the service meets the requirements set out in the Social Care Regulation and Inspection (Wales) Act 2016 (RISCA 2016).
• Collaborate to ensure that the quality of service meets the standards set.
• Responsibility for data collection for service quality reporting.
• Develop, monitor and respond creatively and innovatively to the requirements of the service.
• Be responsible and accountable for the day-to-day management of the service ensuring that all activity meets the relevant legislation, policies and guidelines of Cyngor Gwynedd.
• Collaborate on the development of policies and procedures and review policies relevant to the service.

• Research and ensure an understanding of community resources that exist within the area. Responsibility to share this information so that Support Workers use local resources when supporting children and young people.
• Recruit, appoint, induct, mentor and supervise support staff in accordance with Cyngor Gwynedd's policies and systems.
• Responsibility to ensure appropriate DBS disclosure for all support staff.
• Deal with disciplinary, illness and staff performance matters in consultation with a Human Resources officer.
• Ensure a presence out in the field supporting staff to develop skills and confidence within their role and also to promote and encourage staff well-being.
• Identify the team's training needs and work together to arrange suitable training and doing so in a timely manner.
• Ensure that staff are qualified and understand their responsibilities to register in their professional roles with Social Care Wales in accordance with the requirements of RISCA 2016.
• Create and foster an open culture that enables staff to raise any concerns in an appropriate and timely manner, by ensuring compliance with the Code of Practice for Social Care Workers.
• Responsibility for the administration of the service's credit card, recording, authorising and justifying all expenditures.
• Work collaboratively with other leaders to ensure consistency of arrangements across the county.
• Undertake the role of a champion in a specialist field.
• Follow local and national policies and procedures to identify, record, report and act on any Safeguarding concerns.
• Responsibility for managing and assessing the risk of a variety of community sites used by the service. Be aware of how to report any concerns or adaptations that are required.

• Ensure compliance with Health and Safety rules in the workplace in accordance with the responsibilities noted in the Health and Safety at Work Act 1974 and the Council's Health and Safety Policy.
