Job description
You must demonstrate how you meet the Essential criteria listed in the attached Person Specification in order to secure an interview: How to apply Guidance for applicants Torfaen County Borough Council.We are currently looking for a highly motivated, dynamic individual to join our Adults and Communities Division. The Division provides a service to adults over the age of 18 or in transition to adulthood where they have an assessed eligible need for care and support through the application of the Social Care and Wellbeing (Wales) Act 2014. You will need an in-depth knowledge of and extensive experience in assessing individuals needs applying Continuing Health Criteria accordingly. The primary function of your role would be to support those adults with complex needs through the Continuing Care Process as well as training, coaching and mentoring other fellow Social Workers to do the same. You will need to forge strong links with Social Care and Health Colleagues alike, to produce positive and sustainable outcomes for those with Complex Needs. You will need to and be able to work in an autonomously in often stressful conditions. You will need the ability to present and analyse data.

- Do you enjoy learning, research, legislation and keeping your knowledge and expertise up to date so that you can share it and deliver training?
- Do you have leadership qualities?
- Do you have the skills to Mentor, Coach and "Walk Alongside" Practitioners to enable them to access advice and the level of service through the appropriate assessment and funding routes for the people with whom they work that have Complex Needs?
- Can you work with partners in Health, Education and third sector to achieve positive, equitable outcomes for adults?
