Social Worker - Older People

Assess and manage the needs of older people and their carers, ensuring their health, wellbeing, and social inclusion.

Qualified and registered social worker.
Experience with older people and complex casework.
Strong communication, organisational, and partnership skills.
Commitment to equal opportunities and professional development.
Ability to travel across the county.
What you will do:
Carry out holistic assessments and care management.
Promote independence and person-centred outcomes.
Undertake safeguarding investigations.
Collaborate with other professionals and agencies.
Maintain accurate records and prepare reports.
Mentor new practitioners and contribute to team development.
