Job description

This commitment to positive, strength-based approach is also reflected in our commitment to our staff- to give everyone the opportunity for professional development, gain experience, share their knowledge and reach their potential.We currently have social worker vacancies in the Joint Discharge Team, an integrated health and social care team based in Withybush Hospital. The role of the team is to support safe discharges from hospital ensuring care and support is in place for those who need it. This is also about signposting to third sector or other professionals and organisations where statutory services may not be needed. The successful candidate will need to be experienced in adult care and be competent in completing integrated assessments and care and support plans, as well as an experience and competence in carrying out capacity assessments, best interests and working generally within the Mental Capacity Act. A working knowledge of Continuing Health care would also be helpful.
