The positions are open to recently qualified social workers as well as experienced social workers. The role includes working with adults and carers across Ceredigion who require an assessment within the remit of Social Services and Wellbeing Act (2014) Wales.
Our aim as a service is to support individuals and their families at the earliest opportunity with a focus on recovery and enabling individuals to live as independently as possible. As a social worker in this role, you will have the opportunity to work flexibly and creatively with families adopting a strengths based approach.
Porth Gofal is an integrated service with close working relationships with our local health board and partner agencies, this enables us to work collaboratively and ensure that individuals are supported at the right time by the right service.

- multidisciplinary triaging of referrals to identify the right professional to respond e.g. physiotherapist, Occupational Therapist or Social care workers to ensure right response at the right time
- provide more consistent, coordinated assessment for care and support
- improve the flow of information between services, resulting in faster decisions being made about people's needs and support
- reduces hospital admissions by providing rapid wrap around reablement response service
- provides an in-reach service to reduce delayed transfers of care by providing timely discharges from hospital
