As a key member of our team, you will:
- Undertake strengths-based assessments with adults and carers.
- Develop and implement care and support plans that promote independence, wellbeing, and recovery.
- Work closely with health, housing, and community partners to ensure a holistic and integrated approach.
- Focus on early intervention to prevent escalation of need and support people to live the lives they choose.
What We're Looking For: - A qualified and registered Social Worker (Social Care Wales registration required).
- Experience or interest in working with adults.
