Social Worker (Homeless Prevention) Job description About the role: The role of the social worker for Homeless Prevention is to work with children, young people and their families to prevent homelessness and edge of care. A primary focus of your role would be to engage with 16/17-year-olds who are at risk of homelessness or who are homeless, intervening as early as possible to prevent homelessness and
undertaking joint assessments with colleagues in Housing, in line with the 'Southwark' judgment and when appropriate. Your role will also involve close working relationships with the LA's Housing Department / Homelessness team,with Detached Youth Services and our Early Help team. This will enable you to provide the right help at the right time to children, young people and families where there is a risk of family breakdown, with a specific focis on early intervention. You will developing care and support plans and utilising family and social networks to keep children and young people within their families wherever possible and safe to do so. About you: • Good communication skills, written and verbal. • Good IT skills. • Ability to organise, prioritise and manage a caseload. • Ability to demonstrate a commitment to equal opportunities and no discriminatory practice. • Commitment to the principle of confidentiality. • Must have knowledge and commitment to the principles of social care as embodied in relevant legislation. What you will do: Undertake Care and Wellbeing Assessments, including Southwark Assessments with 16/17-year-olds. Respond to safeguarding concerns in line with legislation when required. Develop Care and Support Plans with young people and their families Work within the Signs of Safety Framework/ work with children, young people and their families to prevent family breakdown and strengthen family relationships / increase stability Preventative work to address risk of edge of care / Family breakdown, including partnership working with a range of other services, including Early Help. Provide guidance, advice and support to staff across children's services around homelessness prevention.
