Job description
30 hours per weekFollowing the requirements of the Social Services and Wellbeing (Wales) Act (2014) you will be working with social workers and other professionals, in a multidisciplinary environment or integrated team. You will work with adults who need support to maintain their independence and wellbeing and help to achieve their agreed outcomes, in addition to supporting carers in their own right. Under the supervision of a suitably qualified practitioner, you may potentially carry a modest caseload or work directly with social workers on their cases, undertaking tasks such as assessments, and planning and arranging care and support, and support for carers. You will work with people in a strengths- based, outcome focused way, focusing on 'what matters' to people and their strengths and capabilities.
