Purpose of the post
• Ensure that the people of Gwynedd are at the centre of everything we do.
• Assess the needs of disabled and children with illnesses and their families / carers in accordance with the relevant statutory requirements.
• Work as a member of a county multi-agency team. Carry out joint duties such as setting up care plans, monitoring and reviewing them to meet needs.
• Assist in the development of new services for disabled and children with illnesses and their families within the County.
Responsibility for functions
• Budget- Provide practical and appropriate advice and guidance to service users to facilitate effective finance management of the service .
• Equipment - Responsibility for any specialist equipment used to perform the job.
Main duties
General Duties
• Practice within the regulatory frameworks and the relevant guidelines which form the basis of the social services provision in Wales.
• Implement the professional code of practice.
• Possess the latest information and maintain your own professional development.
• Take an active part in regular reflective supervision by registered social workers or a registered occupational therapist (depending on the role and tasks) and seek advice and support on work fields beyond the expected work boundaries.
• Manage your own work and be accountable for it and work within the boundaries of the role.
• Collaborate with cross-disciplinary workers under the guidance of your line manager/registered social worker.
• Prepare and submit reports and minutes under the supervision of the line manager / registered social worker by ensuring that the information is correct, consistent and is shared safely and appropriately.
• Support and empower people to participate in decision-making processes.
• Empower people to have a voice and support them to access advocating services.
• Collaborate with people who require care and support and other professional workers.

• Organise and provide care packages to meet with the needs assessed in consultation with the children, their families, and co-workers.
• Conduct relevant risk assessments for the above.
• Monitor and review care plans provided via the multi-agency system.
• Be part of the social work team's duty system.
• Promote links with service providers and relevant Voluntary Organisations.
• Work jointly with parent groups / forums
• Work jointly with the Child Protection Team and the Looked After Children Team in relation to child protection cases and with children who receive accommodation.
• Work jointly with relevant adult teams in terms of planning for transition age.
