Job description
Work base: Coed Pella, Colwyn Bay

Applications are being sought for the position of a full time Section Manager - Commissioned Services via Fixed term appointment or a secondment for 12 months.
You will be required to forge links with health colleagues, Independent Sector and Third Sector providers and to lead effective liaison and negotiations through the Community Resource Teams to ensure delivery of person-centred outcome focused services for Older People in Conwy.
You will have responsibility for the line management and development of the Brokerage, Direct Payment, CHC Team and Financial Assessment Team and will be expected to lead, monitor and evaluate service activity data for all externally commissioned services for older People.
You will also lead on delayed transfer of care from hospitals and will be leading on the development and management of Extra Care Housing in Conwy.
Whilst you will be line- managed by the Service Manager for Older Peoples Services, you will also work with other managers to ensure commissioned services for Adults in Conwy are being delivered and managed effectively.
