Shared Lives is a highly successful way of supporting people to live in their communities as part of a family, having choice and control over their care and support, which has a strength-based approach at its very heart. It is based on relationships which make a real difference to the lives of individuals and their Shared Lives carers.
An exciting opportunity has arisen for an experienced worker to join the West Wales Shared Lives Team. We are looking for a positive, solution-focused and highly motivated individual who strives for excellence in their work.
You will work as part of a friendly, supportive team who are responsible for the recruitment, assessment, and training of Shared Lives carers. We value collaborative working and empowering individuals to shape the support they receive and the ongoing development of the service.
As a Coordinator, you will be responsible for maintaining an agreed caseload of Shared Lives Carers having overall responsibility for the provision of arrangements, matching individuals requiring support to the most appropriate carers and developing person-centred Personal Plans / Individual agreements for each arrangement. You will undertake assessments of Shared Lives Carers for them to retain their approval status whilst liaising with relevant agencies, family members and individuals.
As a Coordinator you will also be responsible for the supervision of Shared Lives Support Workers, the allocation of their caseload and the monitoring/review of their work and personal development.
The role is based in our main office in Carmarthen and will include patch-based work in Carmarthenshire. You will have use of our pool cars for travelling.

As part of the Council's Policy and Procedure, it is now a requirement that employees must register with the DBS Update Service for which there is an annual subscription fee of £16 borne by the individual. The Authority will pay for the initial DBS check.
