Salary – Grade 9, SCP 31 – 35 £41,771 - £46,142
Position – Full Time, Fixed Term.
We are looking to recruit an Occupational Therapist and Social Worker to support a new initiative, the Independence @Home Team (I@HT). The team will work alongside the Newport Community Resource Team (CRT), Reablement Care Team and other core Adult Teams, to provide a dynamic multi – disciplinary opportunity for people to be supported through a reablement pathway that is focussed on:
- What matters and a strengths-based approach of assessment. - A tailored reablement programme of assessment, therapy and support. - Clear reablement outcomes that support working towards independence. - Liaison with your local community support to maintain people at home for longer.
The team will be instrumental in working with professionals to determine if a reablement pathway is appropriate and then will:
- Develop an in depth and focussed programme of reablement support. - have oversight and review people regularly as they move through their reablement programme, to maximise people’s potential and continue to build on strengths. - Support and enable risk management. - Provide good information about the service. - Support people into their community on completion of a reablement programme. - Where people have been assessed as having eligible care and support needs after reablement input, provide information and advice about the next steps and support people with any transfer of care.
The Occupational Therapist role will include building a reablement programme based on assessed functional abilities, that embeds a strengths-based approach to support independence, the Social Work role will include providing key oversight and support for people making enquiries into the team and those within the reablement pathway, to ensure an assessment and review approach is maintained and grounded in a strength - based ethos. Both roles within the I@HT will work with people to enable them to overcome the barriers that prevent them from doing the activities that matter to them; to take opportunities and not to see risk as another barrier, to support and improve an individual’s abilities in a safe way and avoid a focus on what cannot be done in favour of what can be done with greater certainty, accountability and transparency.
The new initiative is a great opportunity for professionals involved to develop and shape the service to determine ongoing requirements.
The posts are on a fixed term basis, initially until 31st March 2027 and secondments with Line Manager approval can be considered.
The Team will have a base in Holly Ward – St Woolos but with the expectation that the team are community facing.
