Job Description
Purpose of the Post.
• Ensure that the people of Gwynedd are at the heart of everything we do. • Gwynedd Council is looking to recruit a committed and compassionate Mental Health Support Worker to join the South Gwynedd Mental Health Social Services Team. • This role involves supporting people with mental health needs to live independently and meaningfully within their communities. Working as part of a multi-agency team, you will provide practical and emotional support to individuals in accordance with their agreed care plans. • The successful candidate will work closely with social workers, social care practitioners and community psychiatric nurses as part of a multi-disciplinary team to support recovery and wellbeing.

• Support individuals with mental health needs to live as independently as possible in the community • Assist individuals to work towards goals identified in their care plans • Provide practical and emotional support to promote recovery and wellbeing • Encourage the development of social and life skills to support independent living • Work in partnership with mental health professionals across services • Maintain accurate records and follow Gwynedd Council policies and procedures
About You
We are looking for someone who: • Has an interest in supporting people with mental health needs • Is compassionate, respectful and committed to promoting independence • Has good communication and teamwork skills • Is able to work effectively as part of a multidisciplinary team • Is willing to undertake training and professional development • Knowledge of the Social Services and Well-being (Wales) Act 2014 and experience of working within mental health services would be advantageous.
