As a Social Worker in Adult Services, you will play a crucial role in supporting adults to live independent and fulfilling lives. You'll manage a diverse caseload, conducting comprehensive assessments and developing personalised care plans tailored to each individual's needs. Working closely with service users, their families, and multidisciplinary teams, you will coordinate care, advocate for individuals' rights, and ensure the effective implementation of support plans. Your work will be guided by the principles of the Social Services and Well-being (Wales) Act 2014, always prioritising the well-being and independence of those you support.
