The IR Service provides reablement to people living in Torfaen to support them to live independently, safely and well in their own homes or within short stay accommodation at Ty Glas y Dorlan. The service is flexible and responsive.
Your role as a Reablement Practitioner offers a unique opportunity to work as part of a multi-professional team. To use your knowledge, skills and attributes to assess, enable, support, supervise and assist individuals following illness and\/or hospital stay to maximise and maintain their independence in a way that is focused on what matters to them and their personal outcomes.
This service operates between 07:00am-23:00 pm, 7 days a week including bank holidays and may involve day, evening or weekend working on a rota basis in line with Torfaen Councils Domiciliary Care Service.
