Are you someone who believes in helping people get back on their feet after illness or injury? This is your opportunity to make a real difference. As an
Intermediate Support Worker, you will support individuals (mainly, but not exclusively, older adults) to recover and regain independence following a stay in hospital, a period of illness, or during times of crisis - enabling them to live safely and confidently at home.
No experience? No problem!If you are caring, calm, and communicate well, with a positive attitude and a genuine desire to help others, we want to hear from you. We will provide all the training and support you need to succeed in this role.
You will be part of a dedicated
Intermediate Care Team within
Carmarthenshire County Council, working closely with nurses, therapists, and other professionals to deliver short-term, person-centred care that makes a genuine impact on people's recovery and wellbeing.
What you will do:- Provide timely care and support during crisis calls, responding to urgent situations where people's health or independence is at risk.
- Support individuals to regain skills and confidence in daily living activities after illness or hospital discharge.
- Deliver personal care (e.g. helping people to wash, dress, or use the toilet) with dignity and respect.
- Assist with meal preparation and medication to promote health and recovery.
- Encourage and enable people to work towards their rehabilitation and reablement goals, helping them return to independence.
- Monitor and record progress, reporting changes in condition and liaising with the wider health and social care team.
