Overall job purpose

Work as an integral part of the Community Mental Health Support Services Team. To provide low, medium and high-level mental health support in accordance with their needs to individuals who are referred to the service in order to maintain or achieve their personal outcome and support their recovery.


This is achieved by identifying and implementing bespoke outcomes that focus on care and support plans for individuals. Emphasis is placed on enabling service users with persistent and severe mental health problems to live their lives as independently as possible in their homes and communities and to be able to acquire and/or maintain safe accommodation and manage their day to day lives and accommodation effectively.
