The Team for Older People have an exciting opportunity for a Social Worker to join an innovative and progressive team.
The successful candidate will contribute to the team's multi-agency approach to providing a seamless health, social care and well-being service for older people, their families and carers. You will work with individuals so that they are enabled to identify their own personal outcomes. Your work will focus on the individual and their support networks including families to promote independence and ensure the individual has a stronger voice in decision making and more control in their lives. The Social Services and Wellbeing (Wales) Act (2014) and Mental Capacity Act 2005 provide legislative frameworks to ensure that the team complies with statutory duties and that individuals are supported to achieve positive outcomes.
The team would like to attract motivated Social Workers at any level of experience. To be the right person for this role, you will have excellent communication skills, proven experience of working as part of a team, previous experience of working with vulnerable adults and the ability to respond appropriately to the needs of older people whilst working within statutory guidelines, policies and standards. The Team for Older People works closely with partner agencies to support and promote safe and timely hospital discharges, so experience of multi-disciplinary working would be an advantage.
