Come and join our team Carmarthenshire Local Authority presents an exciting opportunity for a motivated and experienced Occupational Therapist to join our Releasing Time to Care Review Team.
We are an innovative, well-established integrated review team consisting of Occupational Therapy and Social Work. We strive to deliver the right support, at the right time in the right place as close to home as possible.
As a team, we have an excellent reputation for our recent innovation and leadership to unite, transform and evolve our services according to the changing needs of our population.
This role will form part of the Releasing Time to Care Review Service where you will carry out a review assessment with the aim of providing a less intrusive, more personalised package of care to maximise the individual's independence.
We will work together to provide further assessment, intervention, and rehabilitation to improve function, reduce risk of de-conditioning and improve the well-being of individuals within their home.
The aim of the service is to provide a less invasive package of care for the individual, to improve the lives of people who require support in the community.
