Job Description
Purpose of the post • Ensure that the people of Gwynedd are at the heart of everything we do.
• Promote the person Centred care ethos through standards of professional practice that meets the physical emotional social intellectual and spiritual needs of individuals who use the service
• Support people to make decisions that have an impact on their lives, involving the individuals and families
• Develop and adhere to a personal care plan that is created in partnership with the individual to promote and encourage them to live their lives as fully as possible by making use of any resources and activities which are available in the community.
• Support the individual to live their lives as they want to live it.
Responsibility for functions • Use appropriately any tools and equipment required to fulfil your duties
• Notify your Line Manager and / or the person responsible for the equipment as soon as possible of any defect therein, or the lack of services (water, electricity, gas etc.)
• Ensure appropriate management of service users property e.g. medication and cash if applicable.
Main duties • Promote independence , self -sufficiency , well-being, choice and control by assisting individuals to understand their situation better and help them to develop coping strategies .
• Respect individuals rights, beliefs and choices .
• Support individuals in their wish to stay in their own home within their own communities as long as this is safe and that they maintain their Health , welfare and quality of life.
• Focus on supporting through person centred methods, keeping within the policies and procedures of Gwynedd council and the service.
• To respect the dignity and independence of individuals
• It will be necessary to promote the rights of the individual by identifying, measuring and reducing the element of risk.
• Assist with personal care in identified cases (on a short term basis) if needed.
• The post holder is required to work independently.
• To identify, support the individual to develop their skills and ability, and encourage t the individuals to be actively involved within their community
• The post holder will be expected to work in partnership with the Community Connector to identify social opportunities for the individual.

• Ensure compliance with Health and Safety rules in the workplace in accordance with the responsibilities noted in the Health and Safety at Work Act 1974 and the Council's Health and Safety Policy.

Special circumstances • Will need to go through the Disclosure and Exemption Service process
• Need to work unsociable hours over seven days on a rota basis. This can involve bank holiday and weekends as necessary.
• Dealing with individuals' personal funds in accordance with a care plan or Gwynedd Council's financial transactions guidelines if necessary
• At times there may be a need for the post holder to undertake sleep-in duties
• Carry out duties delegated to you by your line manager
