About the Transition Team The Transition Team supports disabled young people as they prepare for adulthood, ensuring their journey is planned, coordinated, and rooted in what matters to them. We work collaboratively with young people, their families, Children's Services, health colleagues, schools and colleges, and a wide range of community partners.

Ideally you will have; Applicants will need to be social work qualified and professionally registered as a social worker with Social Care Wales. Experienced or newly qualified practitioners are welcome to apply.
You will need a knowledge and working practice of the Social Services and Wellbeing (Wales) Act 2014, to include outcome focused assessment and safeguarding. Also a working knowledge of the Mental Health Measures (2010), Continuing Health Care, the Mental Capacity Act 2005 including Deprivation of Liberty Safeguards.
