Are you ready to be part of a new way of delivering health and social care in Conwy? We're looking for two dynamic, organised, and forward-thinking individuals to join our Community Resource Teams (CRTs) as
CRT Coordinators.This is a fixed term opportunity for 18 months, and we welcome applicants who wish to apply for a secondment opportunity. You must have your manager's permission before applying for a secondment.
About the Role As a CRT Coordinator, you'll be at the heart of our integrated approach to community-based care. You'll work closely with health and social care professionals, voluntary organisations, and local services to ensure people receive the right support, at the right time, in the right place. As a CRT Coordinator, you'll:
- Support CRT Managers and Lead Practitioners in developing integrated, multi-disciplinary working
- Coordinate workflow, meetings, and service planning across teams
- Help implement new systems and support service change
- Liaise with a wide range of partners including health professionals, voluntary organisations, and emergency services
- Collect and analyse data to support service improvement
- Play a key role in shaping the future of community care in Conwy
