Overall job purpose Supporting clients who have mental health challenges; To identify housing support needs outlined in the HSG outcomes framework. To work as an integral part of the Community Mental Health Support Services' Housing Support Grant (HSG) funding team, and to provide low, medium or high housing related support to individuals with mental health support needs (in accordance with identified HSG outcomes' needs) and are living in temporary accommodation or who are homeless or at risk of becoming homeless and are assessed as requiring support to acquire and / or to maintain secure accommodation. To live independently in their own homes and manage their day to day living and accommodation effectively.
To help secure stable accommodation, but also to help retain and maintain existing arrangements and support individual at risk to ensure they do not jeopardize their accommodation status.
The Support Officer will consider identified needs on initial referral form from HSG's SPOA in order to discuss, agree and set and plan outcomes with each individual based on support needs.
The Support Officer will focus on working to achieve successful outcomes for service users engaged with the Service in line with the requirements of the Housing Support Grant programme and to follow HSG guidelines in terms of Rapid Response Housing needs procedures as well as other HSG requirements by the CMHSS.
