As one of the biggest employers in the area, we want you to join our close working friendly team to lead and focus on developing integrated initiatives that enable us to deliver services to the people in Conwy and benefit communities.
The Older People and Hospital Social Work Team within Conwy Adult Services require a full time, Assessing and Reviewing Officer.
This is an exciting opportunity to work within the Colwyn Bay Community Resource Team which is a well-established multi-disciplinary team. You will be working with Older People and their families/carers and also people who have recently been discharged from hospital. You will use a collaborative approach with a strong focus on outcome based planning and reabelement.
Support will be available through regular formal supervision and day to day informal supervision, a variety of supportive group sessions and excellent training opportunities.
To apply for this role you should be able to demonstrate good working knowledge around the social care and/or health care sector and also evidence of further relevant study e.g. NVQ Level 3 or above in Social Care, BTEC Trusted Assessor in Community Equipment.
It is essential that you have the ability to travel throughout the County on a regular basis, often to and from remote locations at short notice.
Due to the nature of the work, it will be necessary to obtain a satisfactory disclosure from the Disclosure and Barring Service (DBS).
