We are seeking a motivated and compassionate Team Support Worker to join our well established and supportive Adult Social Services Intake Team. This is a vital role supporting individuals age 18+ at the front door of services, helping to assess needs and provide timely, person-centred interventions to meet personalised outcomes. The team also delivers an urgent response service to individuals with existing care plans, ensuring that needs are met quickly and effectively to maintain safety and independence.
Working alongside Social Workers and Assessor Care Coordinators, you will support the initial assessment function for people who may have eligible care and support needs under the Social Services and Well-being (Wales) Act 2014. A key part of this role is providing direct, short-term support to individuals, helping them manage their day-to-day activities and achieve positive outcomes. You will work collaboratively with Social Workers, Care Coordinators and health and third sector colleagues to enable person centred outcomes. You will support the delivery of preventative and early intervention strategies by engaging positively with individuals, families, and carers.
We are looking for someone who:
- Has experience in a care, support, or community-based role
- Demonstrates a strong commitment to person-centred and strengths-based practice
- Is able to work flexibly in a fast-paced environment and can respond calmly and effectively in urgent or complex situations
- Has excellent communication and interpersonal skills
- Is passionate about supporting individuals to maintain independence
Experience working in health, social care, or a related field is required along with good organisational and record-keeping skills. A full UK driving licence is essential for this post as the team cover the whole of Pembrokeshire
