We are excited to offer an opportunity for a full‑time, permanent Social Worker to join our Community Learning Disability Transition Team. We are looking for compassionate, motivated individuals with a recognised social work qualification and current registration with Social Care Wales. Whether you are newly qualified or bring years of experience, we welcome applicants who are committed to delivering high‑quality support. In this role, you will manage a varied and meaningful caseload, carrying out assessments and developing care and support plans that genuinely improve outcomes. You will also contribute to CHC applications, Court of Protection processes and, when required, coordinate non‑criminal investigations for Adults at Risk. You will become part of a supportive, skilled Multi‑Disciplinary Community Learning Disability Team who work collaboratively to deliver the best possible service. We value teamwork, reflection and shared responsibility, especially when managing challenging or sensitive situations. We are looking for applicants who can: - confidently assess need and manage risk
- communicate effectively with Young people and adults with learning disabilities, carers and service providers
- work in partnership with parents, carers, agencies and colleagues
- bring empathy, resilience and professional curiosity to their practice
Experience of working with individuals who have a learning disability is beneficial, but we also welcome transferable skills from other areas of social care practice.
