The Older Persons Mental Health Team (OPMHT) is an integrated community service comprising both Social Services and Health professionals, working collaboratively to support individuals with complex mental health needs. As a Social Worker within this team, you will work closely with patients, service users, carers, and partner agencies to assess needs, manage risk, and negotiate outcomes that promote independence and wellbeing. Practice is underpinned by key legislation including the Social Services and Well-being (Wales) Act 2014, the Mental Capacity Act 2005, assessing and managing people as defined by the Mental Health (Wales) Measure 2010 and the Mental Health Act 1983.
The role involves undertaking strengths-based assessments, developing and implementing care and support plans, and coordinating services in partnership with other professionals. You will hold responsibility for managing a complex caseload, acting as Care Coordinator where appropriate, and ensuring that care is delivered in a person-centred, proportionate, and least restrictive manner. A key aspect of the role is balancing the protection of vulnerable individuals with their right to make choices and take acceptable risks, while promoting dignity, autonomy, and inclusion.

The salary for this position is Grade 8 - Newly Qualified £36363 - £39862, Grade 9 Qualified £40777 - £45091 per annum
