Social Worker

About the role

This is a dynamic, frontline social work role within a highly supportive multi-disciplinary team, working alongside Occupational Therapists, Physiotherapists, health colleagues, mental health services, and wider partners.

You’ll manage your own caseload, undertaking timely, outcome-focused assessments in complex situations, making proportionate decisions under pressure, and working collaboratively to achieve the best possible outcomes for people and their carers.

Our work is challenging and time-sensitive, but you’ll never be working in isolation, we value shared decision making, reflective practice, and strong professional relationships across the MDT.

What you will do

In this fast paced MDT environment: 

• Respond rapidly to crisis situations, completing holistic assessments with individuals, families, and carers to stabilise risk and promote independence.
• Work collaboratively with therapy colleagues (OT/PT), health professionals, mental health services, and third-sector partners to deliver coordinated, person-centred interventions. 

• Assess and manage risk confidently, developing proportionate support plans that promote independence and prevent unnecessary hospital admission or long-term care

• Advocate for individuals by navigating access to services, funding pathways, emergency placements, and specialist support, including CHC Fast Track and mental health provision. 
• Apply the Mental Capacity Act 2005 in practice, undertaking capacity assessments and leading best-interest decision-making where required. 
• Contribute to safeguarding enquiries that meet the rapid response criteria, ensuring effective information sharing and MDT involvement. 
• Support carers through timely assessments and joint planning to reduce crisis and sustain wellbeing. 
• Work closely with internal services, care providers, GPs, and the Emergency Duty Team to ensure safe, timely responses and continuity of care. 

What we’re looking for

This role is ideal for a Social Worker who:

• Is HCPC registered with post-qualifying experience in adult social care
• Is confident working autonomously in complex and high-pressure situations
• Values collaborative MDT working and professional curiosity
• Can balance risk, rights, and strengths to achieve proportionate outcomes
• Is resilient, flexible, and motivated by making a tangible difference at times of crisis

The salary for the position is Grade 8 to 9, £36363 - £45091 per annum.
