About the role
This is a dynamic, frontline social work role within a highly supportive multi-disciplinary team, working alongside Occupational Therapists, Physiotherapists, health colleagues, mental health services, and wider partners.
You’ll manage your own caseload, undertaking timely, outcome-focused assessments in complex situations, making proportionate decisions under pressure, and working collaboratively to achieve the best possible outcomes for people and their carers.
Our work is challenging and time-sensitive, but you’ll never be working in isolation, we value shared decision making, reflective practice, and strong professional relationships across the MDT.

- Is HCPC registered with post-qualifying experience in adult social care
- Is confident working autonomously in complex and high-pressure situations
- Values collaborative MDT working and professional curiosity
- Can balance risk, rights, and strengths to achieve proportionate outcomes
- Is resilient, flexible, and motivated by making a tangible difference at times of crisis

The salary for the position is Grade 8 to 9, £36363 - £45091 per annum.
