We are currently seeking a qualified Social Worker to join our Community Resource Team - a large, dynamic, and multi-disciplinary service dedicated to supporting residents across Caerphilly.
This particular role is based within our Hospital Discharge function, where you will play a key role in enabling timely, safe, and person-centred discharges from hospital settings.
We are looking for a motivated practitioner who is confident in strengths-based practice and able to work at pace in a fast-moving environment. Experience within hospital discharge pathways would be advantageous, although not essential. For the role, we ask that you have:
- A Diploma / Degree in Social Work or predecessor qualification.
- Up to date knowledge of relevant NHS policies and procedures e.g. Continuing Health Care etc.
- Ability to construct care plans to demonstrate the required outcomes, minimising risk and meeting individual need.
- Full UK Category B (Cars) driving licence and the use of a motor vehicle insured for business purposes to travel throughout the borough to visit clients and attend meetings.
