The Central Wellbeing Team are currently looking for a highly motivated and dynamic individual to join us at Torfaen County Borough Council, Adult & Communities division. We are a multi-disciplinary team that works together with partner agencies to meet the social care needs of adults from the age of 18. Our aim is to work proactively with individuals, to promote choice, control, and positive risk taking to achieve outcomes in areas that matter most to the individual.
The successful candidate will be a registered social worker who will be based in the Central Cwmbran area of Torfaen. You will be committed to working in a strengths-based style focusing on ‘what matters’ to the individual.
We are looking for flexibility, enthusiasm and dedication to working positively with individuals in the community and hospital settings. You will need to be able to form and develop positive relationships with individuals and carers to ensure that they are supported to achieve outcomes that matter to them.

The successful individual will work as part of a multi-disciplinary team comprising of social workers and a registered mental health nurse.
