About the role
We are seeking to recruit a motivated and child-focused Social Work Assistant to join Porth Gofal, Integrated Triage and assessment team, to work with families and their children. Working within the principles of the Social Services and Well-being (Wales) Act 2014 and Wales Safeguarding Procedures, the role will contribute to the identification of need, risk, prevention whilst ensuring children and their families receive the right support at the right time.
The role will predominantly consist of completing proportionate assessment in line with the referrals, and safeguarding concerns relating to children and families. The post holder will gather and record relevant information, provide advice and guidance, and assist with identifying levels of need, risk, and vulnerability to support informed decision-making. Working under the guidance of the Team Manager and Senior Practitioner, whilst supporting effective multi-agency working with education, health, police, housing, and third-sector partners.
Key functions of Porth Gofal include:
- Providing a single point of access for care and support referrals.
- Undertaking proportionate assessments of need.
- Identifying strengths, needs, risks, and desired outcomes for children and families.
- Providing information, advice, and assistance.
- Signposting to preventative, early intervention, and specialist services.
- Supporting multi-agency information sharing and collaborative working.
- Progressing safeguarding concerns through appropriate statutory processes.
- Promoting wellbeing, prevention, resilience, and positive outcomes in line with the Social Services and Well-being (Wales) Act 2014.
A commitment to possess strong organisational, information-gathering skills, be able to prioritise workloads in a fast-paced environment and maintain accurate records to support informed decision-making is essential. In additional to the commitment to working collaboratively as part of a multi-agency team, providing advice, guidance, and signposting to appropriate services while promoting the wellbeing, safety, and best interests of children and young people.
We are looking to recruit an individual:
- HNC/Nursing Qualification/NVQ4 in Care/QCF National Diploma Health & Social Care Level 5, K101 or equivalent
- Knowledge and understanding of safeguarding children and promoting their welfare, with the ability to recognise and appropriately escalate concerns.
- Experience within a Children’s Services, Triage, Assessment, Early Help, or Safeguarding Team would be advantageous
