Supports adults to maximise independence through strengths-based assessment, enablement planning and review. The role involves managing complex caseloads, working collaboratively with health and community partners, promoting positive risk taking, safeguarding, and ensuring compliance with Welsh social care legislation. Candidates need social care experience, strong communication and assessment skills,
relevant qualifications, and a commitment to continuous professional development.
About you:
The ideal candidate will have strong assessment, communication and problem-solving skills, with the ability to manage a busy caseload and work effectively with individuals, families, carers and partner organisations. They should be confident in undertaking strengths-based assessments and reviews, promoting independence, managing risk,
collaborating with multidisciplinary teams, and maintaining accurate records. Good organisational, IT and safeguarding skills are also essential.
What you will do:
The Social Care Practitioner – Enablement will assess individuals’ needs, develop person-centred support plans, and help people regain or maximise their independence.
They will work closely with families, carers, health professionals and community services, carry out reviews, manage risks, promote safeguarding, and ensure support meets the outcomes that matter most to the individual
