Reablement Support Worker

Our Reablement Team supports people across Pembrokeshire to regain the skills and confidence needed to live as independently as possible following illness, injury or a period in hospital. Working in partnership with Occupational Therapists and other professionals, you'll provide person-centred support that empowers individuals to achieve their goals and reach their full potential.
What you'll be doing
As a Reablement Support Worker, no two days are the same. You'll provide practical support and rehabilitation programmes in people's own homes, helping them to rebuild their independence and confidence. Your role may include:
- Supporting with personal care and daily living activities
- Encouraging mobility and safe movement
- Assisting with meal preparation and medication support
- Monitoring progress and reporting outcomes to the team
- Working towards agreed goals and rehabilitation plans
- Helping to identify long-term care and support needs where required
You'll be part of a supportive multidisciplinary team and will work under the guidance of Occupational Therapists and other healthcare professionals.

Working pattern
We operate a two-week rolling rota across the whole of Pembrokeshire.
Shift times:
- 7:30am to 1:00pm
- 5:00pm to 9:30pm
Week 1: Monday, Tuesday, Saturday and Sunday
Week 2: Wednesday, Thursday and Friday
Part-time hours are available.
As this role involves travelling between customers' homes across Pembrokeshire, a valid driving licence and access to a vehicle for work purposes are essential.
What we're looking for

- Have excellent written and verbal communication skills
- Are compassionate, positive and committed to supporting others
- Can work independently and as part of a team
- Are flexible and able to work evenings and weekends on a rota basis
- Are passionate about helping people regain confidence and independence
An NVQ Level 2 in Health and Social Care (or equivalent) is desirable. If you don't currently hold this qualification, full training and support will be provided to help you achieve it. Registration with Social Care Wales is required for this post.
