The Reablement Team is recruiting.
We are seeking a proactive and confident Reablement Liaison Officer to join our fast-paced and busy service working across Pembrokeshire. This role is key to supporting seamless discharge planning from hospital, ensuring individuals experience a smooth and timely transition into reablement services within the community.
The successful candidate will be responsible for gathering and coordinating essential information, working closely with hospital teams, community services and service users to facilitate safe and effective discharges. You will act as a central point of contact, helping to ensure that people receive the right support at the right time to maximize their independence and recovery. This is a demanding but highly rewarding role within a dynamic environment, requiring excellent organisational and communication skills, the ability to manage competing priorities, and a flexible approach to meet the needs of the service.
The post is full-time, working 37 hours per week, Monday to Friday during core hours. As part of this role, you will be required to register with Social Care Wales, hold a minimum of a Level 2 qualification in Health and Social Care, and demonstrate a willingness to work towards and complete a Level 3 qualification within a timely manner.
Applicants must have a strong knowledge of Reablement principles and practice, be confident in their decision-making and interactions, and be comfortable working within a high-pressure, fast-moving setting where teamwork and adaptability are essential.
