The Information, Advice and Assistance (IAA) Team are looking for a highly motivated and dynamic Social Worker to join us at Torfaen County Borough Council, within our Adult & Communities division.
We are a supportive, multi-disciplinary team working in partnership with other agencies to meet the social care needs of adults aged 18 and over. Our aim is to work proactively with individuals to promote choice, control, independence, and positive risk-taking, enabling people to achieve the outcomes that matter most to them.
The successful candidate will be a registered Social Worker, based at Ty Glas Y Dorlan, Thornhill, Cwmbran. You will be committed to delivering strengths-based practice, focusing on “what matters” to individuals and supporting them to maximise their independence.
We are looking for someone with flexibility, enthusiasm, and a positive approach, who is confident working across both community and hospital settings.
You will work closely with the Integrated Reablement Team, monitoring individuals’ progress and responding flexibly as needs change. At the end of the reablement period, you will work alongside individuals, their families, and partner agencies to review progress against agreed outcomes. You will then take appropriate and proportionate action whether that is stepping down support, arranging ongoing care, or exploring alternative options ensuring all decisions remain strengths-based and centred on what matters to the person.

The successful individual will work as part of a multi-disciplinary team comprising of social workers, enablement leads, enablement support workers, reablement practitioner and social care practitioner.
